The same thing happened to me just before TF2 freaked out on me. When using the Shotgun, the Soldier's right thumb's knuckle was about 8' up in the air. Still flesh colored, but so high up it was off the screen. Then TF2 crashed and I had to physically delete Steam from my hd and reinstall every game. You might want to have someone check that out.
