The brain form is a kind of 'overlord' to all of the flood in one area. You only see it twice, the first time being that big blob that Keyes has become a part of, and the second in the Halo comic featuring Rtas'Vadumee (he kicks ass). Look it up on halopedia.

I think that the laser still is a single beam, but because of the game's engine and graphics, it would appear to be multiple beams when you tried to move the gun while firing it.
edit: thanks for the info on the brain form, I thought it was some new form that had to be fought. (perhaps put in the enemy section which enemies were in the original campaign, so we can tell what CMT added and what was already there.)
another edit: I may be completely wrong, but I think I read somewhere that the Juggernaut forms were the result of failed experimentation with Spartan clones. (and yes, the Spartan clones do exist. If you don't believe me, go read the description for "Chiron TL34" in the multiplayer maps)

Most sources state that the Juggernaut forms are the result of a Drinol becoming infected. I don't see how a human can turn into a Flood without getting infected.

The one picture is from the Halo 3 Comic, and the other is from the IGN Promotional Posters.

Bungie has stated that the spartan laser fires a several beams so its easier to hit targets at a distance. (i.e vehicles)

The SL in that Vid looks sweet, and is an effective anti vehicle weapon instead of a vehicle Flipper W00t!
