A token fee would limit it to serious teams like Rams said. We don't want to have to nullify games because a team dissolves mid-season.
Charging per team is better than per player, since some teams have more players than others but they all impose the same amount of organizational work on the admins. Its a team league, so charge by the team.
Then again this is about fun, and pooling money for a prize may distract from that and give people a reason to cheat.

i like the 2 out of 3 idea. allthough if the teams want to play the third it could be fine, just at least so you win two or one on whatever is agreed.
If the teams cant make it can also go 4on4 or 3on3 on some regular or custom maps as well.
no team slayer for a 5v5 match, its just not a pub.
