The first thing I ever made was some sort of canyon map I don't have pics, but looked exactly like FD04's Heightmap tutorial.

Posted my first map, but first ever mod really was this:



basically I took the car's standard mesh and did a bunch of crap to it and deleted the wheels, then made the windows red :|

Also, the only things I ever made for Halo were PC mods, usually in the interest of making the weapon set as different from stock Halo as humanly possible. I didn't ever do anything groundbreaking, but everybody who tried my mods said they were fun.

As for Source, I've made a couple of box maps for Half-Life 2 but haven't had the patience to fully learn Hammer.
